Ages 21 and under. Provides 28-day residential placement for youth needing a transitional home to continue addiction treatment services. Residence is with a specialized PLEA Family Caregiver who can help access treatment services, prepare for independent living, help obtain employment and other support services. Referrals to this program are made through an addiction counsellor with Vancouver Coastal Health.

Maximum length of stay is ten days. Referrals may be made by schools, doctors, and other agencies or directly by the young person.
